How It Works

Medicare referrals, clearly explained.

Nourish Ability Dietetics accepts Medicare referrals under a Chronic Disease Management (CDM) plan or Team Care Arrangement (TCA). Your GP arranges the referral — once you have it, booking is straightforward.

  • Referral under a Chronic Disease Management (CDM) plan or Team Care Arrangement (TCA)
  • Capped at 5 allied health visits per calendar year under Medicare rules
  • A gap fee applies to your first consultation
  • Review consultations are bulk billed — no additional cost after your first visit

How To Book

A referral, then a simple booking.

A referral from your GP is required. Once you have one, consultations can be booked via Halaxy — available by telehealth or in person by appointment at Mickleham South Community Centre.

2 · Book by telehealth

Video consultations Australia-wide — the most flexible option for busy schedules or patients outside metropolitan Melbourne.

3 · Or in person at Mickleham

By appointment at Mickleham South Community Centre — a quiet, accessible space in Melbourne's north.
